Alcohol / Drug Evaluations (Clinical or Substance Abuse Evaluation)

This type of evaluation is needed in cases where the Court, Probation Officer or other entities require you to undergo an evaluation other than the DUI evaluation. Sometimes this may be done after you have completed the required class hours to see if additional treatment is needed. This evaluation is more in-depth than the DUI evaluation, but uses the same DSM Diagnostic tool.

All of our clinical type evaluations, consist of a set of comprehensive questions designed to determine whether the offender meets the diagnosis of substance abuse or addiction, or has misused the substance that led to the arrest. To determine such diagnosis the Assessment Counselor uses the DSM Diagnostic tool which is also used by the American Psychiatric Society. Depending on the history of the use, previous treatment experience and diagnosis the Assessment Counselor will make a recommendation that will satisfy the requirements of the State where the offense occurred.

Required Information

Supporting documents must be received within one week . If there are special circumstances that time frame can be negotiated at the discretion of the counselor. If you are unable to obtain a document listed below, please contact us to see if your evaluation can still be completed.

  • Notice of Summary Suspensions, or Officers Sworn Report, or any legal document that contains data of your arrest including the BAC or Refusal
  • Police report
  • Drivers Abstract (Driving History obtained from the court not the DMV for court purposes)
  • Copy of Photo ID or Drivers License
  • Print Out of All Previous Offenses (if any)
  • BAC or Lab Work Results (if not refused)
  • *any additional state forms

Additional Fees That May Apply

Depending on your individual situation, the following additional fees may apply:
Some States require additional forms to be completed by the agency providing the services. Please note: We do not have access to forms required by States other than Illinois. If your state requires an additional form to be completed you must obtain it yourself and provide it to our agency.
Special State forms start at $35 - $85 depending on complexity and the number of pages.( This includes progress reports and letters)
Notarization Fee is $25
Express Shipping $35 (overnight via UPS or FEDX)
Rush Evaluation Fee - if you have a court date we can do a same day evaluation.
Missed Appointments $60 (if cancelled within 24 hours of appointment)


Evaluation Types


Alcohol Drug Phone Evaluations $300

This is the standard Alcohol Drug Evaluation done via phone. The evaluation process includes filling out an online questionnaire. Upon making payment you will redirected to the questionnaire. Once you submit your questionnaire and the required documents you will be contacted by the assigned counselor to schedule the phone interview. When all documents have been received and the interview has been completed you will receive the finished evaluation within 2 business days.


Same Day Alcohol Drug Phone Evaluations (Week Day Only) $400

This Alcohol Drug Evaluation is conducted via phone during business hours 9 am - 5pm CST. The evaluation process includes filling out an online questionnaire. The completed questionare must be received by our office no later than 12 Noon CST. Documents must be received no later than 2pm CST. Once you submit your questionnaire and the required documents you will be contacted by the assigned counselor via email to schedule the phone interview. When the interview has been completed and the Counselor has finished the process, you will receive the finished evaluation by 10 pm CST. The completed Evaluation will be emailed to you. Additionally we can fax, mail or overnight it by request (There is an additional charge for the overnight)


Weekend Alcohol Drug Phone Evaluations (Saturdays & Sundays) $500

The Evaluation will be conducted via phone between 9 am CST and 5 pm CST on weekend days only. The evaluation process includes filling out an online questionnaire. The completed questionare must be received by our office no later than 12 Noon CST. Documents must be received no later than 2pm CST. Once you submit your questionnaire and the required documents you will be contacted by the assigned counselor via email to schedule the phone interview. When the interview has been completed and the Counselor has finished the process, you will receive the finished evaluation by 10 pm CST. The completed Evaluation will be emailed to you. Additionally we can fax, mail or overnight it by request (There is an additional charge for the overnight)


Holiday Evaluations (All National Holidays) $600

The Evaluation will be conducted via phone between 9 am and 5 pm CST on any national holiday. The completed questionare must be received by our office no later than 12 Noon CST. Documents must be received no later than 2pm CST. Once you submit your questionnaire and the required documents you will be contacted by the assigned counselor via email to schedule the phone interview. When the interview has been completed and the Counselor has finished the process, you will receive the finished evaluation by 10 pm CST. The completed Evaluation will be emailed to you. Additionally we can fax, mail or overnight it by request (There is an additional charge for the overnight)


Evaluation Updates $110

An Evaluation Update can be done only if the original Evalaution was prepared by this agency and is less than 1 year old and there has been no new offense since then. The process for an Evaluation update is the same as for the evaluation except the Driving Abstract and Police Report are not needed. You will need to submit a new Collaborator form as well as new consents for the release of confidential information for the Court, Probation, your Attorney and your Collaborator.
